Out-of-network dentists may balance-bill you heavily.<\/li>\n<\/ul>\n<p><strong>Humana Dental Mid &#8211; PPO 80%<\/strong><\/p>\n<ul>\n<li>Standard out-of-network protection. Covers up to the 80th percentile of local dental fees if you go out-of-network.<\/li>\n<\/ul>\n<p><strong>Humana Dental High &#8211; PPO 90%<\/strong><\/p>\n<ul>\n<li>Strongest out-of-network protection. Ideal if your preferred dentist is out-of-network.<\/li>\n<\/ul>\n<p>** All three are PPO plans. The main difference is how the plan determines the amount it will recognize and reimburse for a dental procedure. A MAC plan uses a fixed fee schedule, while UCR plans base reimbursement on customary fees charged in the local area.
