Insurance &#8211; District Paid<\/h3>\n<p>The District provides to all benefits eligible employees and eligible TRS retiree re-hires $15,000 in group life insurance and $15,000 in group accidental death and dismemberment insurance.\u00a0 The full cost of this coverage is paid for by the District and will begin the first of the month following 30 days of employment, assuming you are actively at work on that date.<\/p>\n<p>A claim for death benefits must be submitted no later than 30 days after the date of death.\u00a0 Benefits will be paid to the deceased employee\u2019s beneficiary or beneficiaries.\u00a0 Many people choose to name their children as beneficiary.\u00a0 Although it is simple to name your adult child as beneficiary, naming your minor child is more problematic.\u00a0 In most cases, a minor child must have a guardian appointed to manage any payments, and in some cases a trust will be required to receive the payments.\u00a0 The District suggests that employees seek legal counsel in determining their beneficiary designation.\u00a0 Beneficiary designations may be changed at any time.<\/p>\n<p>A claim for accidental dismemberment benefits must be submitted no later than 12 months after the date of loss.\u00a0 Dismemberment benefits will be paid to the employee.<\/p>\n<p>Life Insurance benefits may be assigned to a funeral home for payment of burial or other funeral services.<\/p>\n<p>If your group Life Insurance ceases, you may be able to convert your life insurance to an individual policy.\u00a0 You must apply for conversion within 31 days of the date your coverage ceases.\u00a0 Accidental death and dismemberment insurance is not convertible to an individual policy.<\/p>\n<p>If you are age 70 or older on your effective date of insurance , the amounts of your Basic Life and Accidental Death and Dismemberment Insurance on and after the age 70 will be determined by applying the appropriate percentage from the following table to the amount of your insurance in effect on the day before your 70<sup>th<\/sup>\u00a0birthday:<\/p>\n<\/div><\/section><br \/>\n<div  class='avia-table av-qnr7tv-91fa9bcbf2c2af704245ba76aa322225 main_color
