Transition of Care Form

In effort to make the transition to new Humble ISD/Cigna plan offerings easier on you and your covered dependents, we are requesting everyone who enrolls on the Humble ISD/Cigna plans to review and complete a Transition of Care Form.  If you or your dependents meet any of the criteria outlined on the form, please complete the form, and it will be securely submitted to the medical/pharmacy administrators.

This information is kept completely confidential and is never released to the District.

No services can be denied due to pre-existing conditions.

Why are we asking for this?

1. Assist members who may be in treatment for a serious medical condition.
2. Ensure providers are in network.
3. Nurses at Allegiance can reach out to members and their doctors directly for assistance.
4. Make the transition as smooth as possible.

Pharmacy:

1. The Pharmacy customer service team will aid in the transition of any mail order prescription.
   1. They will help secure all prior authorizations.
   2. They will help secure any manufacturers coupons for high dollar drugs.
