Why Critical Illness Insurance?

A critical illness plan can help with the treatment costs of covered illnesses. Benefits are paid directly to you, unless otherwise assigned, giving you the choice of how to spend the money. Plus, there are plans available to provide coverage for you, your spouse and dependent children.

Continuation of Coverage

This is the process of an employee being able to retain their coverage on a direct pay basis when they leave Harmony Public Schools. To ensure that your portability (continuation of coverage) is handled quickly and accurately, please print the Continuation of Coverage form and then do the following:

  • Complete the form in its entirety and sign and date it.
  • Verify your rate from your paystub or with your agent.
  • If paying by monthly ACH please email completed forms to CSCMAIL@aflac.com.
  • If paying by check, mail the completed form along with your check to the address noted on the form.
  • Submit within 31 days of your termination date.
