Q. Do the cleanings count against the overall plan maximum

A: Yes, cleanings are an expense that is included with the overall yearly maximum.

If you select an Ameritas network provider, you receive access to discounted fees and are guaranteed your dental fee will be no greater than the contracted limits of your plan. For the Low Plan, any out of network fee will also be subject to the Maximum Allowable Charge. Out of network will be enhanced for the High Plan at 90^th% U&C, which means Ameritas will reimburse what 9 out of 10 providers charge in the geographical service area. This may lead to less out of pocket spend if an out of network provider is utilized.
